At XPAND, we provide employees with a truly comprehensive welfare plan that provides flexibility for employees and their dependents. Our plan is a cafeteria style plan that allows you to carve out the benefits that you need. The plan promotes health and wellness while providing financial security for today as well as tomorrow.
Medical Coverage: Employees can select from two Preferred Provider Organization (PPO) options offered through Anthem. All plans offer after a co-payment 100% coverage for most services provided by in network providers. The PPO plans allow for the flexibility to use outside providers at 70% co-insurance less the annual deductible.
Prescription Drug Plans: Employees can select from two Preferred Retail and mail order coverage included in the medical plans listed above.
Dental Coverage: Available as a separate option through a national provider. The plan provides 100% coverage for preventive services, 90% with a $50 deductible for basic services and 60% with a $50 deductible for major services for in network providers. The plan also allows coverage for out-of-network providers at 100% coverage for preventive services, 90% with a $50 deductible for basic services and 60% with a $50 deductible for major services based on usual and customary rates. Basic Life and AD&D Insurance: Core coverage provided by XPAND is equal to one times the base annual salary up to a maximum of $180,000 for eligible employees.
Short-term Disability Insurance (STD): Short-term disability coverage provides you with an ability to earn a living and provide for yourself and for those who are dependent on you if you become sick or disabled (non work related injury). STD would take effect after the employee has been disabled for 14 calendar days. After the 14 day period, this employer paid benefit pays 60% of base salary (to a maximum of $1,000 per week) for 11 weeks. If further coverage is needed after the STD benefit period has been exhausted, Long-Term disability would then become effective.
Long-Term Disability Insurance (LTD): Long-Term Disability (LTD) Insurance provides income protection if you are "totally disabled" and cannot work. This employer paid benefit follows 11 weeks on STD and pays 60% of base salary (to a maximum of $6,000 per month).
